Admission Process

• Initial consultation and assessment

• Development of individualized service plan
• Trial period to ensure good fit
• Completion of enrollment paperwork

Required Documentation

• Child's birth certificate
• Medical records and diagnosis (if applicable)
• Immunization records
• Emergency contact information
• Insurance information
• Previous therapy reports (if any)

Attendance Policy

• Regular attendance is crucial for therapeutic progress
• Please notify us at least 24 hours in advance for cancellations
• Repeated absences may affect service planning
• Make-up sessions subject to therapist availability

Operating Hours

Payment Terms

• Payment due at time of service or monthly in advance
• We accept cash, bank transfer, and insurance payments
• Late payments may incur additional fees
• Non-payment may result in service suspension

Cancellation Policy

• 24-hour notice required for session cancellations
• Late cancellations may be charged full session fee
• Emergency cancellations will be handled case-by-case
• 30-day notice required for service termination

Health Requirements

• Children must be free from contagious illnesses
• Inform us of any medications or medical conditions
• Keep emergency contacts up to date
• Follow our health and safety protocols

Emergency Procedures

• We maintain current emergency action plans
• Staff trained in first aid and CPR
• Parents will be contacted immediately in emergencies
• We may seek emergency medical care if needed

Child Behavior Expectations

• We use positive behavior support strategies
• Aggressive or harmful behavior will be addressed immediately
• Parents will be involved in behavior planning
• Services may be suspended if safety is compromised

Parent/Guardian Responsibilities

• Treat all staff and families with respect
• Communicate openly about your child's needs
• Follow facility rules and policies
• Support therapeutic goals at home

Service Limitations

• We cannot guarantee specific therapeutic outcomes
• Progress varies for each individual child
• Services complement but don't replace medical care
• We reserve the right to modify services as needed

Liability

• Happy Mind maintains appropriate insurance coverage
• Parents/guardians assume certain risks inherent in therapy
• We are not liable for pre-existing conditions
• Disputes will be resolved through local mediation
